Please give 24 hour notice if you cannot make a scheduled appointment so we can open the time for another patient. Otherwise, you may be subject to a $50 fee.

We are currently contracted providers with Anthem Blue Cross PPO, Blue Shield PPO, United Healthcare PPO, Cigna PPO, and Aetna PPO.

We charge for in-person office visits, telemedicine/video visits and telephone consultations as allowed by CMS and your insurance provider.

You are responsible for any applicable co-payments at the time of your visit.

After your insurance company has processed the claim, you are responsible for any unmet deductibles, co-insurance, or non-covered services. Payment is due upon receipt. Outstanding balances after 60 days are subject to a $20 rebilling fee.

If we are not a contracted provider with your insurance, we can submit claims electronically to your insurance provider as a courtesy. However, we will not accept discounted rates.

For patients who do not use insurance, payment is due at time of service, via Visa, MasterCard, cash or personal check.

Please call our office to verify insurance eligibility prior to your first visit.

IT IS THE RESPONSIBILITY OF THE PATIENT TO NOTIFY OUR OFFICE OF ANY CHANGE IN INSURANCE, MAILING ADDRESS, OR CONTACT INFORMATION.
School Forms Policies

Patients must have had a recent Well Child Visit in our office to complete their school or camp form.

You may mail or fax any forms that need to be completed for school, camp or daycare.

Forms brought in during well visits can be completed at the visit free of charge, otherwise there is a $20 charge.

Please allow 7-10 business days for completion. We get many requests at the start of school year - so please plan accordingly.

Urgent requests can be completed in 72 hours for a $30 fee

If your school form does not require a doctor's signature, you can request vaccine information from our office staff and fill in school forms yourself.
Annual Administrative Fee

In order to continue providing high quality and individualized pediatric care, starting in 2024, all families will be responsible for an annual administrative fee.

The fee will be based on the size of the family:

1 child

2 children

3 or more children

This fee will cover non-billed services.

Please discuss with your doctor if this will be a hardship for your family .
