Starting January 2025, all families will be responsible for an annual administrative fee. The fee will be based on the size of the family: 1 child - $200, 2 children or more - $400 

 

Use the links above to pay the annual fee for your family for your primary physician. Make sure to add a note of the patients' last and first names.
