You can pay your bill online using our secure payment pages. 

Please make sure to choose the provider name that shows on your bill. 

All families will be responsible for an annual administrative fee. The fee will be based on the size of the family:

1 child - $200, 2 children or more - $400.  Use the links below to pay your outstanding balance or the annual fee for your family for your primary physician. Make sure to add a note of the patients' last and first names.
